What is social prescribing and how can we help?
Social prescribers are here to help with all social issues that you may have, booking with a social prescriber is easy and can be booked in advance. You can either call the practice or use our online consultation service and request to be booked with the social prescriber
Social prescribers can help & assist you in the following areas:
- Can help with Loneliness and refer you to any local social clubs and activity sessions
- Local community center referrals with people from the same background
- Applying for Benefits
- Housing issues
- Any adaptations required for your home
- Filling out any forms
